Care Coordinator
Summit County Government Public Health Part-Time Care Coordinator (20 hours per week).
This position supports foundational public health services, child passenger safety grant coordination, scheduling health screenings and community events. Necessary skills include MS Office proficiency and knowledge of resources. BA in Human Services or related field preferred. At least one year of experience working with clients. Valid CO Driver license and ability to lift 30 lbs required.
Full Salary Range: $19.68 to $28.96 per hour.
